Essential Job Duties and Responsibilities

Pantry Program & Engagement

Manage Delphi Rises on-site cupboard by purchasing, stocking, coordinating, and replenishing supplies. Maintain a selection of basic clothing items for clients. Assist clients in making selections from the cupboard and provide supportive engagement for those not yet ready for treatment. Prepare meals with individuals in the on-site kitchen, teaching cooking skills, nutrition basics, and meal preparation strategies. Transport clients to farmers markets in a company vehicle. Complete safe food handler certification through Foodlink. Connect clients to other local food pantries for food sustainability after a three-visit maximum at Delphis pantry.

Resource Navigation & Case Support

Conduct standardized HRSN and eligibility screenings using FindHelp and other approved platforms. Identify potential eligibility for SCN services, including housing supports, behavioral health transition services, and reentry programs. Provide navigation assistance such as appointment scheduling, transportation coordination, paperwork completion, and follow-up support. Facilitate warm handoffs to internal and external service providers to ensure seamless care. Community Presence & Support Build trusted relationships with hard-to-reach populations through home visits, outreach events, and direct engagement in community settings. Provide emotional support, model coping skills, and lean in to support clients in the ways they need most. Participate in multidisciplinary case reviews, team meetings, and required trainings. Documentation & Compliance Maintain accurate, timely, and confidential documentation of screenings, referrals, and follow-ups in the electronic medical record. Track all grant-required data. Maintain any professional licensure and certifications, ensuring timely renewals and completion of continuing education.

Qualifications

High school diploma or equivalent required; Associates or Bachelors degree in Human Services, Public Health, Social Work, or related field preferred. Minimum of 1 year of experience in community health, outreach, case management, or social services. Strong knowledge of Rochester-area community resources, providers, and systems of care is required. We highly value lived experience with homelessness, addiction and mental health challenged. Strong interpersonal skills and comfort engaging with individuals in diverse community settings. Basic computer skills, including comfort with data entry and electronic record systems. Bilingual or multilingual skills preferred.
